**Students who are new to the Temple Sholom Religious school are eligible for a two week trial free of cost before committing to a full year of tuition.**

Hebrew Options for 24-25 School Year

Kindergarten to Third grade and/or Beginning Hebrew Students
This brand new cohort will be a parent/student group taught by new Religious School Director, Samara Katz. This Hebrew group will meet for one hour on Sundays after school. Students must have an adult counterpart willing to attend and learn with the student AND the adult counterpart should be the same adult for most classes.

Cost: For the first year of this pilot program, cost will be included in Religious School tuition.



Fourth grade and up and/or advanced Hebrew Students
Jewish Online Learning is a Hebrew program that matches students with real Hebrew teachers via zoom. Together teachers and students go though an individualized curriculum based on the student's current Hebrew knowledge. Scheduling is based on student and teacher schedules. Students new to this program must pass a Hebrew placement test before enrolling.

 
Cost: Returning Jewish Online Learning Students $300
           New Jewish Online Learning Students $400
The cost is normally $1,485.00 per student. $55 for each 30 min Hebrew Lesson, set by Jewish Online Learning.

Consent for Emergency Medical Treatment of 'Child 1'

IF REASONABLE ATTEMPTS TO REACH ME FAIL, I HEREBY GIVE MY CONSENT FOR:
1. The administration of any treatment deemed necessary by the medical professionals I have listed as my child(ren)'s physician in the "emergency contacts" section of this enrollment form.
2. If my designated medical professional is not available, by another medical professional; and
3. The transfer of my child(ren) to any hospital reasonably accessible.

If your child has a food allergy, we require that an unopened EpiPen pack along with instructions and contact phone numbers be provided to the Temple Sholom office in a plastic zip-lock bag.

If there is a chance that your child will need medication, such as an inhaler for asthma, during school hours, please provide the Temple Sholom office  with an unopened package of the medication, instructions and contact phone numbers in a plastic zip-lock bag.

Please enter the name of an emergency contact who is not a Parent/Guardian of the child.
For example:  Aunt, Uncle, Cousin, or friend

Temple Sholom Use of Images in the Media:

Temple Sholom, its employees, or agents have the right to take photographs, videotape, or digital recordings ("Images") of me or my child(ren) and to use these in any and all media exclusively for the purpose of communicating the educational activities of the Temple.  I do hereby release to Temple Sholom, its agents, and employees all rights to exhibit this work in print and electronic form publicly or privately. I waive any rights, claims, or interest I may have to control the use of my image in whatever media used. Child(ren) last names will not be used with media images.
